Your Privacy and Consent

The Share2Care App is designed to help unpaid carers share important information about the person they care for. To do this, we need your permission to collect, store, and share personal information about you which you choose to add to the App.

The Share2Care App enables you to create an online contingency plan. The Share2Care Contingency Plan lets you record that you are a carer, details of the person you care for, and alternative emergency contacts should you become ill or temporarily unable to continue in your caring role. Once completed, the contingency plan creates a flag on your NHS Yorkshire and Humber Care Record to identify you as a carer and securely sends a digital PDF copy of your Contingency Plan to the NHS Yorkshire and Humber Care Record. The Contingency Plan can be accessed through the NHS Yorkshire and Humber Care Record in an emergency by health and care professionals who are caring for you. This will ensure those caring for you can ensure timely and appropriate alternative support is provided to the person you care for.

The Share2Care App also has additional features to allow you to record further information about the cared for person (that isn't uploaded to the NHS Yorkshire and Humber Care Record) but instead remains on the App.

The App will only use your personal information to support the care of the person you care for and to improve communication between carers who agree to join and be part of the Share2Care App Circle of Care for the cared for person. Only your name will be shared with other carers who are part of the circle of care you are part of, unless you choose additional personal information. Your personal data will not be shared with anyone outside of the Circle of Care for your cared for person without your consent.

I consent to the Share2Care App processing my personal information for the purpose of supporting the person I care for including the creation of my contingency plan which will be shared with NHS Yorkshire and Humber Care Record.

You can delete your account at any time if you no longer wish for your personal data to be processed in this way.

The Share2Care App uses essential cookies to keep you signed in and remember your preferences within the app. We do not use advertising or tracking cookies.
